Synths and Robots are the real pain the ass. A human will take 3-4 shots to the torso to kill in medium armor, Super Mutants take 2 - 3x as many shots but are still easy to kill with head shots will weapons like the Hunting Rifle. Weapons and things still scale so that there's still some feeling of getting new and better gear as I level up but most of the enemies are modded to be more realistic. Most human enemies will die in 1 headshot when not wearing armor and most will die in 1-2 headshots even with armor if you're using hi powered weapons and depending on the helmet worn obviously a combat helm is going to handle a pistol round better than a raider sack hood would. I've modded mine to pretty much get rid of the level scaling.
